Seeking a full-time physician assistant or nurse practitioner with a minimum of 1-year of Geriatric experience to join Providence ElderPlace in Portland, Oregon. Here, you will work with a well-established team providing older adults comprehensive care and social services while also managing a small panel of patients at a primary practice location. Be part of a passionate team of care givers caring for older adults throughout the Pacific Northwest.
- Outpatient primary care setting, home visits and visits in community-based settings
- Collaborate with physicians, nurse care manages, social workers, pharmacists, behaviorists, and other team members
- 2-years of post-grad clinical experience working with geriatric population or hospice population preferred
- At least 1 year of Geriatric experience required

Providence ElderPlace is a Program of All-inclusive Care for the Elderly (PACE), and is the first program of its kind in Oregon. With eight conveniently located centers in the Portland area and on the Oregon Coast, Providence ElderPlace provides and coordinates all necessary care services for seniors to remain active within their communities. The PACE model offers medical care, physical and occupational therapy, lab tests and procedures, home care services and more.
