Essential Functions
- Conduct patient visits in the home including medication reconciliation
- Assess existing conditions; diagnose and formulate plan of care
- Write referrals, renew medications, order diagnostics and manage results.
- Complete POLST (Physician Order for Life-Sustaining Treatment) for patients as necessary.
- Advanced directive and advanced life care planning activities are a priority in this position.
Position Details
- Work Schedule: 4 days a week / 7AM-6PM
- Avg Daily Patient Census: 6/day Senior population and some younger patients who are medically disabled and on Medicare supplemental program.
- No Call
- Experience with treating patients with multiple co-morbidities
- Posses advance critical thinking skills
- Strong communication skills
Requirements
- Current Nevada R.N. and N.P. license
- Certificate of completion from an accredited Nurse Practitioner Program
- Current Nevada DEA certificate required prior to start date
- BLS certification
- At least one year experience practicing as an N.P. New grads with a history of seeing patients in their homes or with Geriatrics will be considered
- Must have valid driver’s license and insurance
- Must be comfortable conducting home visits with high-risk patients
- Two years of experience in Primary Care or Hospice
