Essential Functions

• Conduct patient visits in the home including medication reconciliation
• Assess existing conditions; diagnose and formulate plan of care
• Write referrals, renew medications, order diagnostics and manage results.
• Complete POLST (Physician Order for Life-Sustaining Treatment) for patients as necessary.
• Advanced directive and advanced life care planning activities are a priority in this position.

Position Details

• Work Schedule: 4 days a week / 7AM-6PM
• Avg Daily Patient Census: 6/day Senior population and some younger patients who are medically disabled and on Medicare supplemental program.
• No Call
• Experience with treating patients with multiple co-morbidities
• Posses advance critical thinking skills
• Strong communication skills

Requirements

• Current Nevada R.N. and N.P. license
• Certificate of completion from an accredited Nurse Practitioner Program
• Current Nevada DEA certificate required prior to start date
• BLS certification
• At least one year experience practicing as an N.P. New grads with a history of seeing patients in their homes or with Geriatrics will be considered
• Must have valid driver’s license and insurance
• Must be comfortable conducting home visits with high-risk patients
• Two years of experience in Primary Care or Hospice

Preferred 

• Spanish-speaking
